Desiree L. Hoffman 29 of Champaign died at 9:00 a.m. Monday, October 4, 2011 at home.

Memorial services will be held at 10:00 a.m. Saturday, October 8, 2011 at the Church of Christ, 1509 West John Street, Champaign.

Miss Hoffman was born on October 28, 1981 in Champaign, the daughter of Charles Hoffman and Debbie Allen.

Survivors include her father of Rantoul, her mother of Champaign, her children Deja, Anthony and Dae'Vyon Hoffman all of Champaign, three brothers, Shanian Lusk of St. Joseph, Travis Lusk of Urbana and Joe Hoffman of Mahomet, one sister; Angela McNeil of Peoria and her maternal grandparents; Ed and Martha Allen of Urbana.

She was preceded in death by her paternal grandparents and an uncle.

Memorial contributions may be to the Desiree Hoffman Trust for her children in care Busey Bank on Philo Road, Urbana.
